This Telehealth Informed Consent describes the nature of telehealth services offered through the MedVita platform and the rights and responsibilities involved. Please read it carefully before requesting a consultation.
This document is a general template provided for the MedVita website and should be reviewed and finalized by qualified legal counsel and clinical leadership before publication.
1. What Telehealth Is
Telehealth is the delivery of healthcare services using secure electronic communications, such as questionnaires, messaging, and audio or video consultations, between you and a licensed clinician who is not in the same physical location. MedVita’s telehealth services are intended for patients located in the United States, in states where our clinicians are licensed.
2. Nature of the Services
Through MedVita you may complete an online health assessment that is reviewed by an independent clinician licensed in your state. The clinician determines whether treatment, including a GLP-1 medication, is clinically appropriate. Prescriptions are issued solely at the clinician’s discretion and only when appropriate.
3. Benefits and Limitations
Telehealth can offer greater convenience and access to care. However, it has limitations: a clinician cannot perform a hands-on physical examination, and some conditions cannot be assessed or treated remotely. Your clinician may determine that an in-person consultation or additional testing is needed.
4. Risks
Possible risks include technology failures or delays, limited information affecting clinical decisions, and, rarely, security breaches despite reasonable safeguards.
5. Medication Information
Some medications may be prescribed off-label or dispensed in compounded form and may not be FDA-approved for weight management. Where this applies, your clinician will discuss it with you before prescribing. Discuss any questions or side effects with your clinician, and seek emergency care for urgent or severe symptoms.
6. Your Responsibilities
You agree to provide accurate, complete health information, to follow your clinician’s instructions, and to inform your clinician of any changes in your health or any adverse reactions.
7. Privacy
Information shared during telehealth is protected as described in our Privacy Policy and, where applicable, is handled in accordance with HIPAA and applicable state health-privacy laws.
8. Right to Withdraw
Your participation is voluntary, and you may withdraw your consent or discontinue telehealth services at any time without affecting your right to future care.
9. Emergencies
Telehealth is not for medical emergencies. If you are experiencing an emergency, call 911 or go to the nearest emergency room.
10. Acknowledgement
By using the Services and requesting a consultation, you acknowledge that you have read and understood this consent and agree to receive telehealth services through the MedVita platform.
